Definition
The hospital ward turns a bed into a rationing. A character owes it a shift, a secret, a consent; the ward can refuse a diagnosis, a graft, speech. The coat is not the plot: tonight's triage is.
It administers bodies: beds, rounds, families, protocols. A covered error, a graft that will not go, a family that refuses to sign weigh more than a corridor tracking shot.
Unlike a research lab, it does not have months: it has tonight. Unlike an established church, its sacrament is consent — the same landscape violence, another stamp.
What the faction does to the story
- It rations. A bed, a theatre, a graft: someone will not enter, and that someone has a name, a family, an hour.
- It hushes. Medical secrecy, error, donor's name: speaking is a scene, not a posted ethics.
- It consents or does not. A patient, a family, a guardian: three signatures, one life. Refusal has a mouth.
- It internalises admin. Budget, stats, management: care is not alone in deciding, and the disagreement is the shift.
Structure and levers
| Role | Concrete power | Scene lever |
|---|---|---|
| Head of ward | Beds, staff, official speech | Refuse a transfer, cover an error |
| Resident / nurse | Shift, body, rumour | Speak, stay silent, disobey a protocol |
| Administration | Budget, stats, communication | Close a bed, demand a silence |
| Family / patient-relay | Consent, complaint, gift | Refuse a care, threaten, forgive |
Anatomy of the faction
| Layer | Concrete detail | Narrative effect |
|---|---|---|
| Bed | A theatre, a graft, a place that no longer opens | The living is rationed without a Hippocratic speech |
| Secret | Error, donor, a closed file | Speaking costs; staying silent does too |
| Consent | Signature, guardian, a family that refuses | Every care scene is also a law scene |
| Shift | Night, code, a ticking clock | Without a clinical deadline, the ward is only a brochure |
Scenes that work
- The refused bed: a transfer, a graft, a place. Nobody says “you will not pass”; the shift board does it.
- The error one covers: a resident knows, a chief hushes, admin demands silence. To speak is to burn the ward; to stay silent is to let a name die a second time.
- The signature that does not arrive: a guardian, a family, a patient too late. Consent is no longer a form: it is a named refusal.
Pitfalls to avoid
- Corridor scenery — Counter-example: beeps, lights, running coats — and no bed refused. The medical tracking shot does not write the ward.
- The lone vigilante doctor — Counter-example: no chief, no admin, no family. An archetype in a coat remains.
- The oracle diagnosis — Counter-example: an analysis that says everything with no delay or cost. An oracle is not a shift.
- The secret with no door — Counter-example: everyone knows, the file opens nothing. A secret exists if it can silence someone named.
Genre variations
- Medical romance: a shift, a secret, a ward loyalty against a love.
- Thriller: an organ, a ticking clock, a covered error.
- Literary: the body, the shame, a consent one never had.
- War / historical: care under scarcity; the medical rite continues, the grafts do not.
Twists
- The ward protects the guilty: the error is the policy; covering was already a choice.
- The patient holds the graft: the carer has nothing left to refuse; the living has changed levers.
- The secret saves: speaking would have killed the witness; silence was not cowardice.

Revision checklist
- Does a character owe a shift, a secret, a consent?
- Can the ward refuse a bed, a diagnosis, a graft?
- Does a clinical delay weigh tonight?
- Does the secret cost something?
- Do chiefs, carers and admin contradict one another?
